Website privacy

This policy concerns the public website. It is separate from the HIPAA Notice of Privacy Practices governing protected health information.

Information you provide

The public contact form requests a name, callback number or email address, and a general reason for contact. The form intentionally does not provide a free-text field for symptoms, diagnoses, medications, insurance identifiers or other medical details. Submitted contact information is sent to the clinic through its Microsoft 365 email service so staff can respond.

Do not submit medical information through the public website. Use the secure Patient Portal or call the clinic for patient-specific care, records, prescriptions, test results or other clinical matters.

Technical information

The website host and security systems may automatically process technical information needed to deliver and protect the site, such as IP address, browser and device type, requested page, date and time, referring page, and security or error events. Access to these logs is limited to authorized personnel and service providers as needed to operate and protect the website.

Cookies and analytics

The current public-site design does not require advertising cookies, behavioral advertising pixels or third-party audience profiling. If analytics, consent tools or other tracking technologies are introduced, this policy and the site’s controls must be updated before activation.

How information is used

Website information may be used to respond to a request, operate and secure the site, diagnose errors, prevent abuse, maintain records as required, and comply with applicable law. The clinic does not sell information submitted through the public contact form.

External services and links

The Patient Portal is a separate secure service with its own privacy and security terms. The contact page embeds Google Maps, and opening or interacting with the map may allow Google to receive technical information under Google’s policies. Other external links are provided for convenience; their privacy practices are controlled by their respective operators.

Security and retention

Jay Medical Clinic uses administrative and technical measures intended to protect website information, but no internet transmission or storage system can be guaranteed completely secure. Contact requests and technical logs are retained only as long as reasonably needed for their purpose, legal obligations, and clinic policy.

Children

This website provides general information for patients and families and is not designed to create public online accounts for children. A parent or legal guardian should contact the clinic or use an authorized secure workflow for a minor’s medical care.

Policy changes

The clinic may revise this policy when website practices or legal requirements change. The current version and effective date will be posted on this page.
