Accessibility statement
Jay Medical Clinic wants patients and visitors with disabilities to have effective access to its public website and information.
Our accessibility goal
The site is being developed toward conformance with the Web Content Accessibility Guidelines, WCAG 2.2 Level AA. Work includes semantic structure, keyboard access, visible focus indicators, text alternatives, adequate color contrast, responsive reflow, reduced-motion support and clear form labels and status messages.
Testing and improvement
Accessibility is an ongoing process. We work to support keyboard navigation, screen readers, text enlargement, mobile devices, readable contrast, and accessible alternatives as the website changes.
Third-party content
The site links to or embeds services operated by other organizations, including the Patient Portal and Google Maps. Jay Medical Clinic does not control every aspect of those services, but will work to provide an accessible alternative for clinic information when reasonably possible.
Need help accessing information?
If you encounter an accessibility barrier, contact Rhonda McLeod at (850) 675-4546 or rmcleod@jaymedical.com. Please identify the page or information you need and the format or assistance that would be useful. Staff will provide the information through an accessible alternative when reasonably possible.
Communication assistance
For information about language assistance, auxiliary aids, sign-language interpreters, alternate formats and reasonable modifications, review the Nondiscrimination and Language Access notice or call the clinic.
