Understand costs before your visit
Insurance networks and benefits change. Confirm both clinic participation and your plan’s current coverage before receiving non-emergency services.
Verify insurance coverage
Call Jay Medical Clinic and the member-services number on your insurance card. Ask whether the clinic and scheduled clinician are in network, whether a referral or prior authorization is required, and what deductible, copayment or coinsurance may apply.
Self-pay patients
If you do not have insurance, or choose not to use insurance for a service, ask about the expected charge and available self-pay procedures before the visit. Florida law also permits patients to request a reasonable estimate of charges before services are provided.
You have the right to receive a Good Faith Estimate
Under federal law, health care providers must give patients who do not have certain health coverage, or who are not using certain health coverage, an estimate of the expected bill for health care items and services before those items or services are provided.
- You may request a Good Faith Estimate for the total expected cost of any health care item or service, including related costs such as tests, prescription drugs, equipment and facility fees.
- If you schedule an item or service at least 3 business days in advance, the provider or facility generally must give you a written Good Faith Estimate within 1 business day after scheduling.
- If you schedule at least 10 business days in advance, the estimate generally must be provided within 3 business days after scheduling.
- If you request an estimate before scheduling, it generally must be provided in writing within 3 business days after the request.
- If a bill from a provider or facility is at least $400 more than that provider’s or facility’s Good Faith Estimate, you may be able to dispute the bill.
