Before you come in
- Call the clinic to confirm appointment availability.
- Confirm that your insurance plan is currently accepted.
- Bring a photo ID and current insurance card if applicable.
- Bring an up-to-date medication list or use the secure patient process specified by the clinic.
- Bring any forms the clinic has specifically asked you to complete.
Do not submit medical records, diagnoses, medication details, insurance member IDs, or other protected health information through the public contact form. Use the secure Patient Portal or call the clinic.
Forms
Patient forms and medical information should be exchanged through the secure Patient Portal or directly with clinic staff. Do not send protected health information through the public contact form.
What to expect
Clinic staff will provide current check-in, records, billing, and arrival instructions when your appointment is scheduled.
