This website provides general information about Jay Medical Clinic, its location, hours, patient resources and anticipated services. It does not provide medical advice, diagnosis or treatment and does not create a clinician-patient relationship.
Emergencies and urgent needs
Do not use this website or its contact form for an emergency or urgent medical concern. Call 911 or go to the nearest emergency department for a medical emergency.
No guarantee of availability
Services, clinicians, scheduling, hours, insurance participation and website content may change. Call the clinic and verify information before relying on it for a visit or financial decision.
Public contact form
The contact form is limited to general administrative requests. Do not submit protected health information or other sensitive medical details. Use the secure Patient Portal or call the clinic for patient-specific matters.
External services
Links to the Patient Portal, Google Maps, government resources and other external sites are provided for convenience. Those services are operated under their own terms and policies. A link does not necessarily constitute an endorsement.
Acceptable use
Do not attempt to disrupt the website, bypass security, submit malicious code, impersonate another person, scrape personal information, or use the site in violation of law.
Content and ownership
Unless otherwise stated, site text, design and clinic-owned media are intended for clinic use and may be protected by applicable intellectual-property law. Government materials and third-party content remain subject to their respective terms.
Disclaimers and limitation
The website is provided on an “as available” basis. To the extent permitted by law, Jay Medical Clinic disclaims warranties regarding uninterrupted access, completeness and error-free operation. Nothing on this page limits rights or responsibilities that cannot lawfully be limited.
